Forms

The Birmingham Bar Association intends to maintain online a collection of forms prepared by our members in connection with legal matters they have previously handled.

These forms are in Adobe Acrobat (PDF) format, and it will be necessary for you to download and install the Acrobat Reader in order to access them. In some instances, the forms are of the pre-printed variety, which have likewise been placed in Adobe Acrobat (PDF) format. However, with respect to these printed forms, there are a couple of add-ons which will permit you or your staff to complete the forms from within your web browser and print the completed form for filing or mailing. The buttons below will connect you to the appropriate files for downloading. You need to download the reader and plug-in only once.

  • Bankruptcy: These bankruptcy forms have been submitted by Lee Benton, Benton & Centeno, L.L.P.; Landmark Center, Suite 550, 2100 1st Avenue North, Birmingham, Alabama 35203 (205)278-8000. They are subject to the limitations, terms of use and disclaimer contained on the Birmingham Bar Association Home Page.
  • Official Bankruptcy Forms from the United States Bankruptcy Court for the Northern District of Alabama.
  • More Bankruptcy: This Voluntary Petition template, is a Microsoft Word 97 template and was provided courtesy of Sterling L. DeRamus. It can be retrieved from this page and loaded into Word for modification, printing and filing. A second Voluntary Petition containing VBA scripting for automatic completion has also been provided by Mr. DeRamus.   • Civil Forms: The Sterling L. DeRamus Microsoft Word Templates for Civil Actions and Real Estate Transactions. These templates are for use in Microsoft Word 97 and include: A District Court Complaint; an Application for Entry of Default Judgment; Request for Witnesses or Documents for Trial; Subpoena; Subpoena Duces Tecum; Real Estate Contract, Warranty Deed, and Notice of Lien. They can be downloaded to your computer, opened in Word 97, and have the appearance of official pre-printed forms. They are subject to the limitations, terms of use and disclaimer contained on the Birmingham Bar Association Home Page.
  • Corporations: These general corporate forms are presented in HTML format. They may be saved to your hard drive and converted using any of the conversion utilities commonly available or printed from your web browser. They are subject to the limitations, terms of use and disclaimer contained on the Birmingham Bar Association Home Page.
  • District Court forms for post-judgment discovery, fees, and contempt remedies for failure to respond as directed.
  • Domestic Relations: These domestic relations forms are presented PDF format. Some PDF forms may be completed online and printed directly from your browser. However, this is not the case with those in which the lengths of names, addresses and other information varies substantially, as in most pleadings. The Child Support forms required to be filed in any domestic case where children are involved can be completed online and printed directly from your browser. Other pleadings can be printed from your browser for use as a reference in drafting your pleadings. They are subject to the limitations, terms of use and disclaimer contained on the Birmingham Bar Association Home Page.
  • Federal Rules of Appellate Procedure in PDF and Windows Help file formats. Also, Pattern Jury Instructions, civil and criminal are available here.
  • Federal Taxation- Electronic Tax forms Federal & State
  • Worker's Compensation Act - Standard Worker's Compensation forms provided courtesy of John F. Whitaker, Whitaker, Mudd, Simms, Luke & Wells, LLC, in Microsoft Word 97 format: Complaint , Answer, Defendant's Interrogatories, Plaintiff's Interrogatories, Defendant's Request for Production, Plaintiff's Request for Production, Petition for Final Settlement, Order Approving Settlement, Final Decree.
  • Jumbo collections of legal forms from FindLaw.
  • The Administrative Office of Courts maintains a set of comprehensive forms for use in the Unified Judicial System. These documents are likewise in the PDF format and require that the user has Adobe Acrobat resident on his or her system. There are 38 civil, 20 criminal, 9 child support, 4 small claims, 3 traffic and one municipal court forms available at this time. Please check here for these offerings.
  • Elsewhere on the site, we have included generic forms prepared by or required by particular courts or judges. These forms may be accessed by locating the page on the web site which deals with the relevant court or judge and clicking on the appropriate link.
